In atrial pacing, the stimulation artifact precedes the P-wave. In ventricular pacing, the stimulation artifact precedes the QRS complex. Two artifacts are seen if both chambers are paced. The stimulation artifact is larger in unipolar pacing, as compared with bipolar pacing.

Atrial Paced Rhythm This patient has a dual chamber pacemaker (i.e., leads within the right atria and the right ventricle) that is in DDD mode. Currently, the rhythm is atrial-paced (note the pacemaker spikes immediately preceding the P waves) and there is a prolonged PR interval without ventricular pacing.

If spontaneous atrial rate is below the base rate of the pacemaker, then the pacemaker will pace in the atrium. It then awaits activity in the ventricle and, if it does not sense ventricular depolarization within a period of time (see AV delay below), it will also stimulate in the chamber.
